REQUESTS
2002
W2s have been sent out to all persons employed by Dairy Mart
throughout 2002. If you have not received your 2002 W2, there
is a high probability that the form was unable to be delivered
to you due to incomplete or incorrect address information.
If
you have recently moved or changed addresses, please email
a request for your 2002 W2 to jklass@dairymartestate.com.
Indicate "2002 W2 Request" in the subject line and
be sure to include the following Information in the body of
the email:

In
the event that your W2 was received and accidentaly discarded
or misplaced, you may contact CIC Plus at 1-800-360-1265 to
request a duplicate. You will be prompted to enter your social
security number and birthdate. If the system is not accepting
your birthdate, enter January 1st, 2000. The cost to obtain
a duplicate through this service is $12.95
If
you are seeking a W2 from any year other than 2002, please
email your request to jklass@dairymartestate.com.

and be sure to include the following information in the body
