PAYROLL INFORMATION REQUESTS
If
you are a former employee seeking previous payroll information
please email requests to jklass@dairymartestate.com
and indicate "Payroll Information Request" in the
subject line. Be sure to include the following information
in the body of the email:

-Current Mailing Address
-Time Period you are requesting information for. |
If
you are an agency or third-party (Social Security Department,
Current and Prospective Employers, Unemployment Department,
etc) seeking payroll information in regards to a former employee
of Dairy Mart, please email requests to jklass@dairymartestate.com.
Indicate "Third-Party Payroll Information Request"
in the subject line and be sure to include the following information
in the body of the email:
