401k PLAN INQUIRIES

The 401(k) plan was terminated in December 2002. The plan has received a favorable determination from the Federal Government and is proceeding towards a final distribution to plan participants. Please download and review the IRS Tax Model Notice for important information to help you decide how to receive your Plan benefits. After reviewing this document, please fill out the 401(k) Distribution Request Form to register for the distribution. Completed forms should be mailed to the following address for processing:
